Instructions:

  1. Copy link into browser.
  2. Download form to your computer.
  3. Once the form is downloaded, you will be able to edit the document. 
  4. Once you have completed the form please email the form to nallen@amanda.k12.oh.us or drop the form off to the school.

CCP Guidelines (How to become a CCP student)
